An employee was cleaning an air station with a pressure washer when the water stream went through their boot and contacted their left foot, resulting in a 1.5 inch by 1 inch laceration to the top of the foot.

An employee was operating a forklift in reverse carrying five bundles of plywood. Upon applying the brakes, the top load shifted backward onto the top of the forklift cage and pinched the employee's the left middle fingertip against the forklift cage, resulting in a partial fingertip amputation.

An employee was testing whether electricity was available at a valve on a piece of equipment that moved from side as it cut logs. After testing, he inserted an electric plug back into the valve. This caused the equipment to move to the side and then return to its home position. The employee was caught between a motor and a hydraulic tank and suffered an injury to the left shoulder and upper torso, resulting in three broken ribs.

On June 17, 2024, at approximately 5:55 a.m., a machine operator injured his right leg after stepping into a 6-inch gap between sections of the edger infeed platform grating. The employee sustained a muscle tear and hematoma.

An employee was helping repair a sawmill trimmer. The employee transferred a pry bar from their right hand to their left hand, then turned, at which point the employee's left middle fingertip was pinched between the pry bar and a saw box. The fingertip was crushed at the nailbed, but not fractured; the employee was hospitalized and required surgery.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

On May 9, 2025, an employee was using an air hose to clear debris from the courtyard area when she slipped and fell. The employee was hospitalized with closed fractures to her left fibula, tibia, and medial malleolus.

On April 24, 2025, an employee was opening a cylinder door on a hydraulic bridge to remove lumber when their left middle finger was caught between the door and the hydraulic bridge, resulting in an amputation without bone loss.

Two employees were using a plywood chop saw. The injured employee was holding one end of the saw with their right hand when the operator on the other end pressed the start button. The saw blade contacted the injured employee's right hand, resulting in multiple finger amputations when it amputated the hand above the thumb.

On January 17, 2025, at 12:15 p.m., an employee was moving 11-gauge steel sheets from a holding cart using a pendant-controlled, 1-ton jib hoist equipped with a plate clamp lifting attachment. With the plate clamp empty and open, the employee began raising the hoist. The clamp became caught on the bottom of the cart, tipping the cart in the direction of the employee. Several sheets of steel slid off the cart and struck the employee, resulting in several broken ribs, a broken left leg and ankle, a fractured kneecap, and a torn left ACL. The employee was hospitalized.

An employee was operating an air hoist to rotate a blending mill counter-clockwise. After he connected the hook to the mill, the hoist was raised and his hand was pinched between the hook block and the ring on the mill. The employee sustained a partial amputation of his right thumb.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
