Federal OSHA recorded a severe workplace injury at PEARCE FOUNDRY, INC., 16099 AIRLINE HWY, PRAIRIEVILLE, LOUISIANA 70769 on — Fractures and burns, affecting the multiple body parts, n.e.c..

An employee was transporting steel casting flasks from one area on a finishing floor to another area using a forklift. One of the turnbuckle posts ripped from the flask causing the load to shift on the forks of the forklift. Both flasks then rolled off the forks and pinned the employee underneath them. This also caused the flask coverings to separate from the flasks and the employee was covered in approximately 700 degree sand from the chest down. The employee suffered burns to his legs and fractures to his pelvis.

AMERISTAR APARTMENT SERVICES, L.P.

On October 26, 2023, an employee was riding on the forks of a forklift that was moving a stack of glass panels. The forklift made a turn and the glass fell on the injured employee, causing lacerations and crush injuries to their hands.

SMITH FLOORING

An employee was on a tractor and moving a stack of lumber when a board shot out from the stack and struck the employee's right leg. Their tibia was fractured and required surgery.

Jillamy Packaging and Warehouse

An employee was using a powered industrial vehicle (PIV) equipped with specialized horizontal clamps to move multiple heavy paper rolls. The top roll came loose from the clamps, falling towards the cab of the lift and lacerating the employee's middle and ring fingers.

Postel Erections Group, L.L.C.

An employee was helping a forklift operator move a steel joist. The joist struck a forklift tire, and as the forklift started to move, the joist shifted and struck the employee's right leg, resulting in a fracture.

AAA Cooper Transportation

An employee was using a pallet jack to move product from a truck to a customer location. A box on the top of the pallet shifted while the pallet was going over the bump of a dock plate and fell on the employee s head. Afterward, the employee experienced numbness in their limbs.

An employee was unloading metal from two drums on a pallet. The employee removed the lid from the second drum and the drum fell over onto his ankle. The employee sustained a fractured ankle.

An employee was working with team members on the pouring line, casting a sand mold. Molten metal entered the employee's personal protection suit and severely burned his left leg, calf, and ankle.

Northern Stainless Corporation

An employee was jointing a board with a jointer. The board jumped when it came into contact with the jointer's cutter head, and the jolt caused the employee's left hand to slide between the board and the guard and come into contact with the cutter head. He suffered lacerations/partial amputations to the middle, ring, and index fingers.

MetalTek International, Inc.

An employee was turning off a gas valve when he bumped into a die brush and struck the spinning die. The die pin struck him on the back, resulting in contusions, abrasions, and a severe laceration to his back that required hospitalization and surgery.

Staley Steel LLC

At 11:45 a.m. on June 13, 2025, an employee of Staley Steel, LLC, was performing preventive maintenance on an overhead crane. While he was holding a grease gun hose and nozzle, he contacted electrical components inside an electrical box. He suffered a first-degree electrical burn on the palm of the left hand, as well as pain up his left arm to the shoulder, and was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

CHRISTUS St. Frances Cabrini Hospital

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

Jean Simpson Personnel Services, Inc.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
