A forklift was lowering a 7,500-pound motor onto timber cribbing. The timbers shifted and the motor moved, then slid off the forklift blades. It pinned an employee against a hydraulic skid, breaking the employee's left femur and right leg before the knee.

An employee was leaning down to install conduit. When he stood up, a 2-inch all-thread rod caught and lacerated his left eyelid.

An employee was removing a 24" OD actuator valve that weighed 10,000 pounds from a structure on the fifth floor using a lattice crane. During the lift, the valve became hung up on the permanent handrail and their left thumb was caught between the valve and the handrail. The employee sustained a partial amputation of the thumb.

A crew was using a chain fall system to rig up a hard stop (metal structural support weighing 695 lbs.). The injured employee was guiding it into position. The hard stop shifted and their right hand was caught between the suspended hard stop and the adjacent structural wall, resulting in fractured ring and little fingers.

An employee was walking back from the tool room through the scaffolding material storage area. The employee's pant leg got caught on one of the 8-foot long scaffold planks that was in a scaffold rack. The employee tripped on the scaffold plank and fell forward, landing on his left elbow. The employee sustained a compound fracture to his left arm above the elbow and was hospitalized.

A crew was using rope to move a 24" diameter blind flange from the receiving deck of elevation 4 to a lower location so it could be more easily accessed by a crane. The rope broke and the blind flange fell 15 inches to the receiving deck of elevation 3, landing on the injured employee's feet. Both feet were fractured.

Star Pipe Products LTD

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

KP Building Products, Inc.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

BJ'S WHOLESALE CLUB, INC.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

International Steel and Counterweights LLC

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

Seneca Mechanical, LLC

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

SK Construction LLC

An employee was fixing a broken purlin at a worksite when they fell from the framing to the concrete floor approximately 19 feet below. The employee sustained brain bleeding and fractures to their face, vertebrae, and wrists.

Employees were removing scaffolding that was used to repair an ion exchange tank. The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

Samsung E&C America, Inc.

An employee was moving a ramming machine to a staging area. While using the controls on the side of the machine, there was a sudden heavy rainfall causing the equipment to slide. The ramming machine tipped over and caught the employee against the ground. The employee sustained a fracture to the right side of their pelvis and was hospitalized.

United Dairy Farmers

The injured employee was training a new employee on a dough machine. She went to show where the batter comes from and her right index fingertip was cut. The employee sustained an amputation to the fingertip. The machine was not guarded at the time of the incident.

MVA CONSTRUCTION LLC

An employee was operating a roller machine on a dirt slope when the machine hit a bump and fell onto it's side. While trying to get free of the falling machine, the employee's legs were pinned by the top bar of the cage. The employee was hospitalized and his left leg was amputated. He also sustained fractures to a hand and finger.

Kisatchie Midnight Express

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

CHRISTUS St. Frances Cabrini Hospital

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

Jean Simpson Personnel Services, Inc.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
