On January 4, 2021, an employee was holding two runs of duct work that were inserted into a tabletop corner insertion machine when the machine activated by proxy switches that sense the presence of the metal and begin the sequence. An angle fixture popped out of the table and traveled from a horizontal to a vertical position to hold the duct work while a corner clip was being applied. The employee's gloved left ring finger was pinched by this angle fixture, resulting in an amputation.
