An employee was getting wood out of a shaver box when his foot was caught in the shaver head. Five of the employee's toes on the right foot were amputated.

An employee was installing a belt over a pulley on a fan when his left middle finger was caught between the belt and the pulley. He pulled his hand out and tissue was removed from the first joint of his finger.

An employee was outside blowing off a concrete pad when he experienced nausea due to dehydration (heat stress), requiring hospitalization.

An employee noticed that a shaver box was making a strange noise and called a coworker for assistance. After the coworker identified that the noise was being caused by a piece of wood that was hung up in the shaver box, he pressed the stop button to initiate the shutdown process. While the employee was attempting to remove the piece of wood, his foot was on the chain of the shaver box. The chain was still coasting to a stop and pulled the employee's right foot into the drive sprocket. He suffered a hole in the top of his foot, a crushed fourth toe, and a deeply lacerated pinky toe. His right fourth toe had to be surgically amputated.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was operating a saw when their right hand contacted the blade(s), resulting in amputation of four fingers and a partial thumb amputation.

An employee was using a telehandler to move pallets of pellets from a pod. The employee exited the telehandler to pick up a sacrificial pallet that is placed under the bottom layer of pallets. When the employee turned to pull the pallet away, the top two tiers of the pod fell on him. The employee sustained a fractured ankle, three fractured ribs, and fractured vertebrae.

An employee was loading a piece of wood onto a machine when the wood came out from under the guard and struck their arm, resulting in a fractured arm.

An employee was preparing to replace a belt on a fan drive assembly. He went to manually stop the fan and was caught by the belt. His hand was pulled into the machine and his left little fingertip was amputated.

An employee was cutting remanufactured lumber with a chainsaw when a board fell and struck his left hand, causing his left hand to move off the chainsaw. While he was moving his hand back, the chainsaw kicked back and lacerated the top of his left hand. The employee was hospitalized.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
