An employee was installing a belt over a pulley on a fan when his left middle finger was caught between the belt and the pulley. He pulled his hand out and tissue was removed from the first joint of his finger.

After the coworker identified that the noise was being caused by a piece of wood that was hung up in the shaver box, he pressed the stop button to initiate the shutdown process. While the employee was attempting to remove the piece of wood, his foot was on the chain of the shaver box. The chain was still coasting to a stop and pulled the employee's right foot into the drive sprocket. He suffered a hole in the top of his foot, a crushed fourth toe, and a deeply lacerated pinky toe. His right fourth toe had to be surgically amputated.
