A maintenance employee was working on the outfeed of a palletizer that wasn't running. He used a wrench and chain puller to make repairs. Once running, the equipment pulled his left hand into a chain and sprocket drive. The employee suffered a fracture to the middle finger, a laceration to the ring finger, and partial amputation of the little finger.
On February 23, 2022, an employee was operating an automatic palletizer machine. The employee approached the chain conveyor to adjust a crooked slip sheet on a pallet when their pullover sweatshirt sleeve was caught in the moving chain conveyor. The employee slipped on the concrete floor while attempting to free the sleeve from the chain conveyor and fell, sustaining a fractured left arm above the elbow. The employee was hospitalized.
