Final narrative
A maintenance employee was working on the outfeed of a palletizer that wasn't running. He used a wrench and chain puller to make repairs. Once running, the equipment pulled his left hand into a chain and sprocket drive. The employee suffered a fracture to the middle finger, a laceration to the ring finger, and partial amputation of the little finger.

The employee approached the chain conveyor to adjust a crooked slip sheet on a pallet when their pullover sweatshirt sleeve was caught in the moving chain conveyor.
