At about 11:30 a.m. on February 17, 2021, an employee was clearing cardboard pieces from a flatbed truck. He began to slip on a piece of cardboard and jumped off the truck landing on the concrete surface about 6 feet below, suffering a right leg fracture above the ankle. He was hospitalized.

An employee was troubleshooting the crimper action of a machine that made press paperboard cylindrical containers. As he passed a photo eye in an interlock gate area, the machine cycled and the indexing arm struck him on the head/face, crushing him against the chuck. He suffered a broken jaw, loss of teeth, and injuries to the back of the neck and head.

An employee was removing a part from a bending machine. The machine clamp amputated the bottom tip of the employee's right little finger.

The operator entered information into the data screen of the line winder, then turned around and walked to the paper rolling machine on the line. He tripped on the edge of a presence sensing safety mat and fell into the machine. The employee's left arm struck a bar and his right hand landed on an in-running nip point of the vertical roller, horizontal feed belt, and the housing of the vertical belt. Three fingers on the operator's right hand were degloved.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was walking behind the rewind coding machine while on his phone. He rested his left wrist on top of the slitter bar and his finger came in contact with the roller, resulting in amputation.

The injured employee was assisting the bander operator with dressing units with dunnage. A unit became stuck on the conveyor. The injured employee was pushing the unit on the conveyor when a conveyor roller pulled his shoe in-between the rollers. The employee sustained crushing injuries to their left foot/toes.

An employee was inspecting and adjusting a tray in a thermoforming machine when the the trigger switch was touched, causing the machine door to close on their right hand. The employee sustained crushing injuries to the hand.

An employee was adjusting the tracking of a conveyor belt when their left hand/arm were pulled into the conveyor. The employee sustained fractures.

An employee was working to retrieve debris from a powered conveyor when the rollers contacted his right thumb, resulting in a laceration.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
