An employee was working in the basement when lithium batteries in his pocket caught fire, resulting in burns to his hip and the top of his leg.

An employee was moving a metal door, creating a spark. The employee's boots caught on fire due to residual flammable chemicals that were on his boots. The employee sustained burns to the ankle/metatarsals of both feet.

An employee was lubricating a forming machine when hot bottles fell off the machine. A bottle contacted the employee's left leg and ignited their flame retardant pants. The employee's left leg was burned from the thigh to just below the knee.

An employee was returning from their break when the rags in his back pocket contacted a nearby stove heater. The rags caught on fire and spread to the employee's clothing. He sustained third-degree burns to the left and right buttocks.

An employee was pouring iron powder into a blender. The powder ignited and burned the employee's left hand, as well as a finger on his right hand.

An employee was climbing a ladder when he fell to the ground. He was hospitalized with injuries to his right knee and leg and required surgery.

An employee was performing regular job duties, including bussing tables. The employee was punched in the face by a patron of the restaurant. The employee was hospitalized with loss of consciousness, brain swelling/bleeding, and multiple avulsions of teeth.

An employee was putting food items on shelves in a freezer. The employee lost their footing while moving around in the freezer and fell on the floor. The employee sustained a fractured hip.

A person wandered through a restaurant into the kitchen area. While being escorted to the front doors by two employees, the person pulled out a sharp object and stabbed one of the employees in the lower abdomen. The employee was hospitalized.

An employee was standing on a milk crate to clean the grill. The milk crate slipped and the employee's right hand fell onto the hot grill, resulting in burns to the right arm and hand.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
