On May 16, 2024, at 3:25 PM, an employee was loading guardrail components onto the forks of a forklift in an outdoor area when he slipped and fell to the ground, resulting in a fractured left leg that required hospitalization and surgery.

Two employees were riding in a utility task vehicle when a front wheel locked up during a turn. The vehicle turned onto its side, and the employee who was a passenger suffered a broken leg and required surgery.

An employee was pulling wet grain out of a grain vac and came in contact with the air lock (fan blade) resulting in the amputation of two fingertips.

An employee was repairing a tire on a piece of machinery. The employee was attempting to pry rubber back to replace a seal when the prybar slipped and caused the tip of his left index finger to be pinched between the two-piece metal rim, resulting in the amputation of flesh.

An employee fell from a water truck's working platform to the ground 43 inches below, striking their head on the concrete floor. The employee suffered broken ribs and a brain bleed.

An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

An employee was attempting to remove a tarp from a flatbed trailer when the trailer handle struck the employee in the chest, resulting in two broken ribs and a punctured lung.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was preparing to unjam an auger shaft while standing on a forklift-elevated, job-made platform. The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.

An employee had been cleaning a grain bin. He walked across the bin and his left foot contacted an unguarded floor auger, resulting in severe lacerations to the foot. The auger was not locked out/tagged out at the time.

On September 23, 2025, an employee was monitoring a feed roller when they tripped and their right hand contacted the roller. The employee was hospitalized and their right hand was amputated at the wrist.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

A skid loader backed over an employee's leg, breaking it.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
