An employee was using a forklift to set a racking system upright that had been knocked over. The rack caught him, knocked him down, and pinned his right hand against the concrete. He suffered a broken thumb and tendon damage. The employee was hospitalized for surgery.

On September 21, 2021, an employee had finished cleaning the floor and cabinets inside a newly manufactured home. She then started descending a distance of 4 feet from the manufactured home floor to the production floor using a 6-foot A-frame ladder. She fell and hit her head on the production floor. She was found unconscious with bleeding from the head. She was hospitalized with a head injury.

An employee was grabbing onto a crane track while standing on a scaffold when the wheel moved and amputated his left pinky and ring fingers.

An employee was cleaning the inside of a finished mobile home. She was using a two-step stool to get off the home when the stool moved, resulting in a broken left ankle. She was hospitalized.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

On September 5, 2025, an employee was installing gang plates on a structure when the side walls fell and hit her head. She then fell onto her back and the side wall pressed her to the ground, resulting in hospitalization due to an ilium fracture and a transverse process fracture on the right side.

An employee was blowing cellulose insulation into the open attic area of a manufactured home. While the employee was holding the insulation hose, it caught on the guardrail of the catwalk/suspended walkway leading into the attic. The tugging motion caused the employee to lose their footing and step onto the sheetrock ceiling. The employee fell through the ceiling to the ground about 8 feet below and suffered a broken lower left leg. The employee was hospitalized.

An employee was going between a ladder and a wall to repair a wall in the utility room. A brad-nailing gun was hanging on a piece of wood on the side of the ladder. The employee contacted the brad nailer and the trigger was pushed into the wooden piece. The brad-nailing gun actuated and shot a 2-inch brad into the employee's abdomen.

An employee was on a scaffold caulking and painting a manufactured home. She fell off the scaffold an estimated 6 to 8 feet, landing on the concrete floor. The employee suffered upper body injuries and was hospitalized.

An employee was going up a ladder from the first level roof to the second level roof. The ladder slipped and the employee fell approximately 8 to 10 feet to the surface on the first level. The employee sustained fractures to three ribs and bruises on their arms and hands.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
