An employee was working on catalytic converters. During the process of opening the catalytic converter, a shear bar pinched his fingers, amputating his right middle and ring fingertips and crushing his right index fingertip.

An employee was cutting a catalytic converter with alligator shears. While the employee was trying to prevent the converter from falling, the clamp actuated. The employee's right index finger was crushed between the converter and the clamp and amputated at the first digit.

On December 6, 2023, an employee of Duke Energy was working on a single-phase 120-/240-volt parallel service re-tap when a secondary flash occurred in an underground service. The employee suffered a second-degree burn to the face and was hospitalized.

A piece of metal was fed to a conveyor system and it struck another piece of metal, causing a piece of titanium (1-inch-long and 1/2-inch-wide) to break off and strike the employee in their upper chest below the clavicle. The employee was hospitalized and the embedded piece of metal was surgically removed.
