An employee was putting a machine guard back onto a chain and sprocket when their left middle finger became caught in the chain and sprocket. The employee sustained an amputation of the fingertip.

On December 3, 2024, at approximately 11:30 PM, electronic technicians were adjusting a tensioning chain on an elevator on a palletizer that had become unlevel. After loosening the tensioners and creating slack in the chain, the injured employee was placing the tensioning chain back onto the sprocket. While moving the chain it regained tension and their left middle fingertip was caught between the chain and the sprocket and was amputated.

An employee was cleaning jammed potatoes from the top of a roll sizer in the raw receiving area when her right index finger was caught in the chain, resulting in hospitalization and a fingertip amputation.

On December 12, 2023, at approximately 3:00 a.m., an employee was unplugging a drain with a powered snake tool when his left hand was caught in the snake cable. The cable pinched his left ring finger, resulting in a fracture and partial fingertip amputation.

On September 8, 2023, an employee was on break when they slipped on the floor and fell on their right hip. Their hip was fractured.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was painting a wall. They fell down three steps and landed on a plastic mop bucket and the floor. The employee was hospitalized with fractures to their right arm and right hip, and required surgery.

An employee was using an electric jack to load a pallet onto a truck when he was struck from behind by a forklift, resulting in a laceration to his right leg.

An employee was performing maintenance on a tray packer. They put a piece of cardboard down onto the belt of the tray packer and stood on the belt. The belt started, causing the employee to lose balance and fall approximately 2 feet. The employee was hospitalized with a fractured ankle and required surgery.

An employee was adding cilantro to the dicing machine when it became jammed. While clearing the jam, the blade amputated approximately 1/2" to 3/4" of his right middle finger.

An employee was cleaning and sanitizing a mixer when the machine was activated. The employee's hand was caught in the machine and he sustained the amputation of his right hand. The employee was hospitalized.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
