An employee was discharging concrete into an excavator bucket. The bucket moved and struck the employee's right ear, crushing and lacerating it.

As he was removing his finger from the tank and simultaneously stepping down from the work platform, he ring was caught in the tank and the left middle fingertip was amputated. He was hospitalized.
