The employee was hospitalized with a head laceration, a fractured left clavicle, fractured ribs, and a possible collapsed lung.

The injured employee was pushing a tilt cart down a main aisle when a riding propane forklift collided with the cart. The employee fell and struck their head on the floor. They sustained a skull fracture and a right elbow contusion.

An employee was retrieving an empty blue bin. When he turned around, he tripped over a skid and fell to the floor, striking his head and sustaining injuries to his hip and foot. The employee was hospitalized with a fractured left hip.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
