A temporary employee was sitting on a chain at the loading dock and planning work tasks. The chain broke and the employee fell off the loading dock, landing on the pavement outside of the building. The employee was hospitalized with a head laceration, a fractured left clavicle, fractured ribs, and a possible collapsed lung.

A forklift struck an employee from behind as it backed up. The employee suffered a broken left leg.

On August 27, 2017, an employee inside a powder spray booth was reaching out to touch the controls when the employee fell approximately 6 feet below, breaking the left hip.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was helping offload an aircraft. She was standing in the opening between a parked cart and an unoccupied aircraft tug when another vehicle struck a stationary bag cart, which then struck her. The employee suffered a skull fracture near the right eye, a right kneecap fracture, a nose fracture, and sinus damage, as well as soft tissue damage to the left calf and hip. She was hospitalized and required surgery.

An employee was cutting steel channels from a hanging rack that was being supported by a forklift. When they were finished, the forklift started moving the rack out of the area and a piece of material snagged one of the rack legs, causing the rack to tilt backward. The rack slipped off the forklift and struck the employee on his right side/lower back. He sustained a fractured pelvis and was hospitalized.

The injured employee was standing by one of the stationary computers along the wall. There was a half-pallet located approximately 3 feet behind the employee that consisted of 12-pound boxes stacked six boxes high. A forklift was transporting a pallet that came in contact with the stationary pallet behind the injured employee, causing four boxes to fall and contact their legs. The injured employee was hospitalized with leg injuries.

On January 7, 2025, an employee was walking to the timeclock when they tripped on matting sticking out from a workstation and fell to the ground. The employee was hospitalized with a fractured right hip.

An employee reported to work at 2:00 p.m. and conducted a pre-shift inspection on a stand-up powered industrial vehicle (PIV). The employee was standing on the PIV with their hands over their head when they fell 10 inches from the forklift platform onto the concrete floor, resulting in injuries to their back and to the back of their head.

An employee was performing a die change on a 150-ton press when he began to feel weak, suffering from possible dehydration and/or heat exhaustion.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
