Final narrative
On June 28, 2021, at around 9:00 AM, an employee was coming down a wood stairway while carrying some materials. He slipped and fell down about 6 to 8 treads to the ground below, fracturing his lower left leg.
Employees were on the roof to install a generator. The injured employee went into the attic and fell through the attic landing on a table and then falling to the floor. The employee sustained back injuries.
Two employees were changing air filters on top of a roof. They had accessed the roof by way of a scissor lift. As they started to descend down, the scissor lifts' wheel rolled into the dirt and the lift fell over. The employees fell about 20 feet to the ground.
