Final narrative
An employee was getting a tarp crank handle out of the way by pushing the handle behind the ledge of a roll-off box. He then turned to pick up a chain that was on the ground to hook to the box. Tension on the handle caused it to release and hit the employee in the head/face. He was hospitalized with a forehead contusion and a laceration to the bridge of the nose.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

The bolt that attaches the chain to the handle of the chain tongs broke, causing the chain to swing around and strike the employee's lower right leg. The chain severely punctured his leg and fractured his tibia.

This caused him to lose balance and fall off the truck bed with his leg still caught in the hole. As a result, he sustained fractures to his left leg.
