A maintenance employee was unjamming a golf straightener. When the machine was jogged, the employee's finger was caught between the carrier block and the frame and crushed, resulting in amputation.

At about 2:45 p.m. on August 22, 2022, an employee was using a hand grinder to grind down a weld bead on a shaft. The grinder cut through the bead, and the change in momentum caused his left hand to contact the grinder wheel. He suffered a laceration to the back of the hand between the thumb and index finger, and was hospitalized. The grinder was not guarded at the time.
An employee was reinstalling an agitator pump at the edge of a heated liquid salt tank (600 degrees F). The bottom of the pump rests on a mount and the pump is attached to it with bolts. The alignment tool slipped out of the bolt hole and the employee's left hand contacted the heated liquid salt resulting in burns to the knuckles on his hand.
