At about 2:45 p.m. on August 22, 2022, an employee was using a hand grinder to grind down a weld bead on a shaft. The grinder cut through the bead, and the change in momentum caused his left hand to contact the grinder wheel. He suffered a laceration to the back of the hand between the thumb and index finger, and was hospitalized. The grinder was not guarded at the time.

An employee was reinstalling an agitator pump at the edge of a heated liquid salt tank (600 degrees F). The bottom of the pump rests on a mount and the pump is attached to it with bolts. The alignment tool slipped out of the bolt hole and the employee's left hand contacted the heated liquid salt resulting in burns to the knuckles on his hand.

An employee was adjusting twisted lifting chains on the end of the pipe when the pipe was raised, causing the employee's hands to be caught between the chains and the pipe. The employee suffered injuries to the left index finger, left ring finger, right ring finger, and right index finger; the right index fingertip was amputated without bone loss.

During a routine inspection, an employee placed their hand on a pipe to check a weld. The pipe clamp activated, closed on the pipe, and trapped the employee's right middle finger, resulting in a partial amputation and fracture to the fingertip as well as a laceration to the ring finger.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.
