An employee was setting up an anodizing line and adding sulfuric acid via a pipe delivery system used in the anodizing tanks. When the employee opened the delivery valve on the sulfuric acid piping system, a valve fitting failed, causing sulfuric acid to spray onto the employee. They sustained chemical burns to the chest, stomach, lower neck, arm, and leg.

An employee was adjusting part placement on a machine. The press head actuated and pinched the employee's left thumb, causing a laceration and amputation to the tip.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
