At about 9:35 a.m. on July 12, 2021, an employee was using a rebar bender. The machine actuated while the employee was holding a bar with two hands; its pin rotated, and the employee's fingers were caught between the pin and the rebar. The employee suffered fractures of fingers on the left hand and was hospitalized.
