Federal OSHA recorded a severe workplace injury at Non-Ferrous Extrusion, 1100 Industrial Blvd., CALDWELL, TEXAS 77836 on — Fractures, affecting the chest, except internal location of diseases or disorders.

An employee was hanging overhead water lines while inside a forklift box attachment approximately 14 feet off the ground. The box attachment fell to the ground and the employee suffered broken ribs.

The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.

An employee was on an extension ladder installing a satellite dish on a roof when they fell approximately 12-15 feet to the ground. The employee sustained fractures.

An employee was setting a 5-foot pry bar into a precast concrete slab and stood on the end of the pry bar to move the slab into place. The pry bar slipped out causing the employee to fall backward 15.5 feet to the dirt ground. The employee was hospitalized with a broken leg.

An employee was on a step ladder to measure and mark a crane rail for modification. The employee fell approximately 12 to 15 feet to the ground, resulting in a fractured right hip.

An employee was on a trailer securing a load of logs when they fell approximately 9-12 feet to the ground. The employee sustained brain hemorrhaging and lacerations to the thigh and above the eye. The employee was hospitalized.

Hydro Extrusion USA LLC

An employee was conducting preventative maintenance on an extrusion press and using an impact wrench to remove rivets on the shear cylinder. When the last rivet was removed, the stored energy in the cylinder released and dropped it onto the employee's hand. Their right thumb was caught between the cylinder and the casing of the rivet gun, resulting in crushing injuries. The employee sustained an amputation of the thumb without bone loss. The machine was locked out/tagged out at the time.

An employee was operating a gasoline-powered pump to remove water from the ground floor sump of a storage building. The pump became blocked and overheated. The employee was removing the blockage when he was sprayed with hot mud and steam, resulting in first- and second-degree burns to his right hand and left wrist, as well as a third-degree burn to the underside of the left wrist.
