An employee was driving an electric riding pallet jack. His left foot slipped off the jack and was crushed between it and metal racking. His left tibia was broken.

An employee was backing up a motorized pallet jack. The employee started to step off the pallet jack while it was still moving backwards and her right leg was pinned between the pallet jack and a floor scale. The employee sustained two broken bones in the lower right leg.

The technician sustained a fracture and laceration to the cheek, and required surgery for an injury related to his right eye. He was hospitalized.

An employee was cleaning and performing maintenance on a vertical panel saw when the employee impacted the saw blade. They sustained a finger fracture and laceration.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was restocking a refrigerated area of a warehouse when he slipped on condensation and fell to the floor, resulting in a fractured hip.

An employee was operating a double pallet jack. While he was pushing a pallet back, the pallet jack moved and the handle crushed his finger against racking.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
