An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was switching valves on a pump in the recovery boiler area. While en route to the equipment, the employee entered an area where excavation work was being performed and that was flooded with 6 inches of cloudy water. The employee stepped into an inspection port that was waist high and sustained burns to the lower body from a hot water mixture.

An employee was tagging a lower bundle when a bundle of cardboard weighing approximately 1,200 pounds fell from above and onto the employee who sustained multiple fractures and was hospitalized.

An employee was refinishing a roll of paper in the refinishing area. The employee manually pushed the roll onto the slat conveyor, however the roll continued on, striking a barrier and causing the roll to bounce back to the slat conveyor. The roll then rolled back to the barrier, striking the employee in the left arm and fracturing the ulna.

An employee was working on a turbine to connect a dehumidifier to a generator unit. A localized flash-fire occurred resulting in burns to both of the employee's arms and a contusion on the left side of his head.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Tools and parts were being lowered to the ground from an elevated platform in a 5-gallon bucket. The rope being used to lower the bucket failed; the tools and parts fell and lacerated the left forearm and upper left leg of an employee on the ground.

An employee was tracking an infeed belt on a scrap hogger when their right arm was caught between the belt and a roller. The arm was broken and the employee's shoulder was dislocated. The employee was hospitalized.

An employee was working to clear a jam from a paper napkin folder. The ram that advanced the napkins pushed the employee's right arm into the side of the machine, causing several lacerations and a hairline fracture.

An employee was troubleshooting overflowing ink on a printer-slotter machine when their right ring fingertip was caught in a guarded roller, resulting in a partial amputation.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
