An employee was driving a skid loader when it fell through the wooden walkway of a bridge and struck the ground 23 feet below. The employee suffered broken ribs, a broken shoulder, and a spinal burst fracture, as well as biting through his bottom lip.

An employee was using a dozer to clear a plot of land. The dozer struck the ground after getting stuck on a tree stump, causing the employee to hit their head on the cab roof. The employee sustained a spinal compression injury.

On August 31, 2023, an employee was operating an excavator to complete a demolition job. As the excavator drove into the first floor of the building, the floor collapsed. The vehicle and employee dropped approximately 8 feet, causing the employee to sustain a back injury.

An employee was driving a forklift to deliver lumber to a work site when the forklift tipped forward onto the forks. The employee struck his head on a metal railing on the forklift, resulting in a laceration, a concussion, and temporary loss of consciousness.

An employee was ascending a hopper car. As he neared the top of the car, he grabbed the hand hold on a running board. The running board shifted, causing the employee to fall to the concrete shop floor. He sustained a fractured left hand and dislocated fingers.

On July 15, 2024, an employee was climbing the fixed side ladder of a railcar while performing a visual inspection when they missed a grab bar on the ladder, falling to the ground and suffering a fractured right femur.

An employee was removing a forklift from a trailer when the lift fell sideways onto the ground and pinned his right leg underneath it. The employee suffered a broken tibia and fibula and other injury to his lower right leg that caused an amputation below the knee.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.
