An employee was unjamming a forming machine when her fingers contacted the rollers that pull wire through the machine, resulting in a pinched ring finger and injuries to the little finger that required amputation down to the middle phalange. The machine was not guarded at the time of the incident.

While reaching for the spacer with his other hand, the control panel was inadvertently pressed and the pinch rolls closed, causing his left index finger to get caught between the upper and lower mounting blocks. The employee sustained an amputation to the fingertip.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
