An employee was sanding a conduit pipe inside a CNC lathe. The employee's left hand and arm became entangled in the running clamp of the machine and the employee sustained a broken left forearm.
An employee was winding fiberglass onto a spinning mandrel when his gloved right hand was caught by the fiberglass/mandrel and pulled into the winder. When he pulled his hand out, his middle and ring fingertips were amputated.
