An employee was working to reposition a spacer located between pinch roll mounting blocks on a slitter machine. He was standing to the side of the control panel with one hand on the control switch. While reaching for the spacer with his other hand, the control panel was inadvertently pressed and the pinch rolls closed, causing his left index finger to get caught between the upper and lower mounting blocks. The employee sustained an amputation to the fingertip.

Employees were performing cleaning duties throughout the plant using hydrochloric acid and caustic water. The employee sustained second and third-degree burns to the right leg.

On October 28, 2017, an employee was walking to a forklift when he tripped over the safety bumper, which is on the floor of the facility as a safety measure to separate forklift traffic from the pedestrian walkway. The employee fell to the floor and injured his right knee and left elbow.

An employee was sanding a conduit pipe inside a CNC lathe. The employee's left hand and arm became entangled in the running clamp of the machine and the employee sustained a broken left forearm.

An employee was unjamming a piece on a shape pultrusion line. Once the piece was trimmed back, an employee realigned the second puller on the line so that the cured piece could pass through the second puller. When the cycle was engaged, the machine amputated the tip of the employee's right thumb.

An employee was operating a hoist to lift material. His left index finger became pinched between a D-ring (lifting device) and the material being lifted, resulting in an amputation of the fingertip.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
