An employee was working to reposition a spacer located between pinch roll mounting blocks on a slitter machine. He was standing to the side of the control panel with one hand on the control switch. While reaching for the spacer with his other hand, the control panel was inadvertently pressed and the pinch rolls closed, causing his left index finger to get caught between the upper and lower mounting blocks. The employee sustained an amputation to the fingertip.
