An employee was adjusting the butterfly tip on an airless paint gun. The gun injected air and pressurized paint into the employee's left index finger, causing a laceration.

An employee was descending a structural fixed ladder when the employee lost grip and fell approximately 10 feet to the lower level. The employee suffered fractures to the right and left ankles.

A Brock employee was installing insulation in or near the vent drum of an aerator when hot water condensate released out of a steam line directly above the employee. The employee sustained second degree burns and was hospitalized.

An employee was standing in an area where paint was stored, when a flash fire occurred. The employee suffered flash burns to her torso, arms, legs, and cheeks.

An employee was monitoring the loading of a railcar when he noticed a vent was leaking. He tried to close the secondary valve to isolate the leak. He put his safety harness on and pulled a probe up to close the valve and isolate the leak. When he pulled the probe up, he heard a pop. The vent hose blew off, spraying him with acetic acid. He suffered first and second degree burns to his lower legs and feet, requiring hospitalization.

The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.

An employee was on an extension ladder installing a satellite dish on a roof when they fell approximately 12-15 feet to the ground. The employee sustained fractures.

An employee was setting a 5-foot pry bar into a precast concrete slab and stood on the end of the pry bar to move the slab into place. The pry bar slipped out causing the employee to fall backward 15.5 feet to the dirt ground. The employee was hospitalized with a broken leg.

An employee was on a step ladder to measure and mark a crane rail for modification. The employee fell approximately 12 to 15 feet to the ground, resulting in a fractured right hip.

An employee was on a trailer securing a load of logs when they fell approximately 9-12 feet to the ground. The employee sustained brain hemorrhaging and lacerations to the thigh and above the eye. The employee was hospitalized.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.

An employee was assisting the crew with patching and restoring a roadway after the installation of a sewer line. He was helping place and level the blacktop/backfill material over the trench area to bring the road surface back to proper grade. The employee was walking between a forward-moving dump truck and a dirt pile when his right foot was run over by the truck s wheel, causing him to fall. The employee sustained injury to his foot.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

An employee was operating a skid steer with a bucket attachment at a demolition site. While driving it to the rear of the property to park it, the employee came into contact with a live electrical transformer unit that was located outside of the facility. The employee sustained electrical burns on his hands extending up to his elbows and a fractured ankle.

An employee was installing a door on a semi-trailer. They were using a forklift to lift the door to the height of the trailer when the door became jammed between the forklift mast and the hinge on the trailer. When the employee went to dislodge the door from the mast, the door fell and crushed their foot. Their big toe was amputated. The employee was hospitalized and their big toe was surgically reattached. The employee's foot and other toes also required surgery.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
