An employee was troubleshooting a miscellaneous box strapping machine. Their left index finger was caught in a component on the tabletop of the machine. The employee's finger was partially amputated.

An employee went to the cafeteria after completing their work shift. The employee slipped and fell on the floor, fracturing their patella.

Two maintenance employees were unplugging a hair chute on a 9' high bin. One employee accessed the chute through the access door. Hair and air blew out the access door and knocked the employee backward off the top of the hair bin to the concrete floor. The employee sustained fractures to both wrists. The machine does not have guard rails and fall protection was not in place at the time.

An employee was setting up their work station for the day, slipped and hit their head on a nearby conveyor. The employee was hospitalized for an eye contusion and later lost their right eye.

An employee was leaving a cooler area on a pallet jack after dropping off a pallet of material. The employee's lower right leg was pinned and broken between a rack beam and the pallet jack housing.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was scraping dried glue from the top lid of a hot glue pot. His left hand slipped and came into contact with the hot glue. He then suffered burns to his hands and fingers as he tried to remove the glue. He was hospitalized.

An employee was in a scissor lift approximately 20 feet above the ground to place a plastic tarp on a catwalk in order to catch condensation. They positioned the lift opposite the work area and stepped out of the scissor lift onto an I-beam. The employee fell from the I-beam to the ground. The employee was hospitalized with a skull fracture on the right side of their head and injuries to the chest and both eyes.

A maintenance employee was called to look at a pallet conveyor machine where a chain had fallen off a sprocket wheel. When the employee grabbed the loose chain, a light sensor caused the machine to restart. This created a pinch point between the chain and sprocket that caught the employee's left little finger. The finger was surgically amputated at the second knuckle.

The injured employee was standing by one of the stationary computers along the wall. There was a half-pallet located approximately 3 feet behind the employee that consisted of 12-pound boxes stacked six boxes high. A forklift was transporting a pallet that came in contact with the stationary pallet behind the injured employee, causing four boxes to fall and contact their legs. The injured employee was hospitalized with leg injuries.

An employee was using a box strapping machine to place straps on boxes when his right middle finger was caught in the machine, resulting in an open fracture to the distal phalanx.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
