An employee climbed the back of a grain trailer to help direct loading. Upon reaching the top, he fell off the back of the trailer. He suffered fractures to the left and right wrists, a head laceration that required staples, a lower back injury, and a brain bleed.

An employee was descending a portable ladder after filling a customer's fuel barrel, lost balance, and fell to the gravel ground. The employee sustained a compound fracture of the left ankle that required hospitalization and surgery.
