On October 4, 2021, at 8:20 a.m., two employees were using a hand pallet jack to move a trash dumpster to a crane bay. At the base of the trash hopper, there are two slots and a skid that allow a forklift to move the dumpster. Between the slots is a small metal pedal that allows the dumpster to slide forward. The employees attempted to insert the pallet jack under the skid, but the metal pedal was obstructing the forks of the pallet jack. Employee 1 reached down to lift and hold the pedal into position as Employee 2 inserted the forks. The forks pinched Employee 1's left middle finger, resulting in a fingertip amputation.

An employee was removing the top bolt from a VSV actuator. The actuator pivoted on its lower bolt and struck his left foot, breaking bones.

Employees were removing hydrogen seal casing using a c-beam to rig it out of its position. The work group had the seal free and floating while attempting to prevent it from being damaged. The c-beam made contact with the end shield without the employees realizing it. They verbally gave a crane operator the signal to cable down, and when he did, the seal came toward the group with more force than expected. The seal casing hit one employee's right hand on his little finger, resulting in a fingertip amputation.

An employee was moving a cart with a grey board on it. Due to shifting of the cart or a wheel catching on an obstruction, the cart tipped over and struck two employees. One employee sustained a laceration on his arm requiring hospitalization.

An employee was assisting with cutting a vertical steel pile. A 16-foot by 9-inch section of pile dropped to the side and bounced off a previously cut pile and toward the employee. The employee was struck in the torso and leg, causing his knee to dislocate from his femur. The employee was hospitalized.

On July 30, 2025, an employee was utilizing a circular saw to cut plastic into strips that would be used as shims. The blade of the saw amputated his left little finger to the second knuckle.

An employee was working on the ingot loader of a aluminum processing machine and noticed a pin was not installed. The employee reached to retrieve the pin and the machine activated for a functions test. The machine closed on the employee's hand, resulting in a hand injury requiring hospitalization.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
