On February 19, 2025, an employee got off their forklift to pull a box from a racking system. After getting the box, the employee stepped backward and tripped over the forklift forks. The employee fell and sprained their back.

On October 8, 2021, an employee was walking to a furnace when he tripped over a spud bar approximately 18 inches off the ground and fell into a batch charger, resulting in head wounds on their forehead and scalp that required staples and stitches.

On August 9, 2019, an employee was working at his forming station. The employee started to feel ill, went home and was then hospitalized with heat stroke and kidney failure.

An employee tripped and fell over the charging cable to her forklift, suffering a femoral neck fracture of the left femur and left shoulder pain. She was hospitalized and had surgery.

On February 27, 2018, an employee was using a bench grinder when the employee's right hand was pulled into the grinding wheel, amputating the middle fingertip.

An employee was taking a trash cart to a dumpster when one of their fingers was pinched between the trash cart and the dumpster, resulting in a partial amputation.

An employee was pushing flat shopping carts back into the cart tunnel. When unjamming the carts, part of the cart struck the employee's left shin, resulting in a laceration and bleeding. The employee was hospitalized.

An employee was pulling a cart backward in a storage area when his right little finger was pinched between the cart frame and an electrical box mounted to the side of a support column. The employee's right little finger was partially amputated.

An employee was helping to push a welding trailer out of the path of a truck. The trailer then rolled over a half-inch elevation change that rocked the trailer toward an electrical box. The employee's finger was pinched between the welding trailer and electrical box, resulting in a finger amputation approximately 1/2-inch from the end.

An employee was unloading packages from a trailer onto a cart. While moving the cart, it struck the employee's heel causing a tear to their Achilles muscle. The employee required surgery.

On September 1, 2024, two employees were dislodging the latch of a hopper to dump a load of glass fragments. The injured employee stopped to walk around to the other side. The other employee freed the latch and the load of glass fragments was dumped, pinning the injured employee to the ground. The injured employee was hospitalized with a fractured left femur.

An employee was walking when they fell and struck the front of their head.

An employee was adjusting the vacuum control valve on the right side of an A-frame arm when their finger was caught between the vent blow pipe and the control valve. The employee's right middle finger was partially amputated.

An employee was opening the milling machine when they began experiencing respiratory issues such as a running rose, coughing, and eye itching from the milling machine vapors. The employee was hospitalized.

An employee was rolling a table with glassware product loaded on it. The table collapsed onto the employee, who suffered a broken lower leg.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
