An employee was using an air powered spray gun to apply peelable paint to a unit when they dropped the spray gun. The employee tried to catch the spray gun and hit the trigger, causing it to spray through their glove and into their right thumb.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

Weldship Corporation

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was walking on the top deck of a fire truck that was being assembled. The employee lost their balance and stepped on open, horizontally hinged doors, which could not support their weight. The employee fell 6 feet to the concrete floor and suffered fractures to their elbow and pelvis.

An employee was assessing an overhead stamping press door for alignment issues when the counterbalance moved and their left thumb was caught between the chain and the sprocket. The thumb was amputated. The door was locked out/tagged out at the time but gravitational pull was not controlled.

An employee was performing a bubble test (pressure test) on a pipeline to locate a leak when the joint elbow that was producing the leak detached and struck him. The employee sustained fractures to a rib and clavicle, as well as injuries to his spleen and lung.

An employee was installing a combat vehicle turbine engine to a dynamometer stand in a dynamometer test engine cell. While lining up the turbine engine to the stand, the employee's left arm was fractured between the engine and the face of the stand.

CHALLENGE MANUFACTURING LLC

An employee was operating a forklift to retrieve a stack of empty containers from behind a weld cell. The forklift grabbed the stack. Then the forks tilted back, causing the top container to fall onto the forklift cage. The employee got off the forklift and went to pull the container down. The container fell on the employee's head and leg, resulting in a bump to the head and a fractured right leg.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

Foundation Steel, LLC

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
