Two employees were replacing a seal on a press machine. A maintenance employee was using a forklift to raise/lift the seal while the injured employee was standing on a ladder using a piece of angle iron as leverage to replace the seal. As the maintenance employee lifted the forks of the forklift to seat the seal in place, the injured employee's left middle finger was caught between the seal and the angle iron, resulting in a fingertip amputation.
