An employee was performing maintenance on a pre-wash cold end washer. As he pushed down on the assembly so that it could be fastened in place his hand slipped downward into a chain and sprocket on the washer's conveyor. The equipment was not de-energized or locked out. His right index finger was caught in the chain and sprocket resulting in amputation of the fingertip.

An employee was using a flip dolly to rotate a rack of glass. The employee used their right hand to operate the flip dolly's wire control box when the dolly rotated downward and pinched their right thumb, resulting in a partial amputation.

An employee was pushing a metal hopper when the wheel became caught in the grating. The hopper tipped over and struck the employee in the leg, breaking the employee's tibia.

An employee was seriously injured after being pinned between a shuttle and conveyor belt while attempting to adjust the glass breakout arm. The employee suffered fractured ribs with a possible spine injury.

An employee's right arm got caught in a conveyor belt system while the employee tried to remove a piece of glass that was stuck on the conveyor belt. The employee's right arm contacted a rotary pulley, causing a laceration to the bicep area. The employee was hospitalized for surgery.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was operating a machine and went to remove a buildup of desiccant from the machine. Their right index finger was pinched between two moving parts, resulting in an amputation to the finger.

On September 3, 2025, an employee was working on the left-side charger of a tank. They were using a ratchet with an extension to make an adjustment to the sand seal when their right thumb was pinched between the ratchet and the running charger, resulting in an amputation at the first knuckle.

A forklift was placing an elevated load onto a truck. The forklift tipped forward and fell on the injured employee who was standing nearby. The employee sustained fractures to their hip and ribs, as well as leg muscle injuries.

At 12:09 p.m. on July 11, 2025, an employee was inspecting the dust collection system in a plant. The employee was checking an airlock to determine whether there was suction when their left index and middle fingers were pinched between the rotating rotor and the housing of the airlock. The employee suffered amputations of both fingertips without bone loss.

An employee was manually removing a bearing from a vertical washer brush. Due to the weight of the brush roll, it released and crushed his left ring fingertip resulting in amputation without bone loss.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
