An employee was manually removing a bearing from a vertical washer brush. Due to the weight of the brush roll, it released and crushed his left ring fingertip resulting in amputation without bone loss.

An employee was operating a machine and went to remove a buildup of desiccant from the machine. Their right index finger was pinched between two moving parts, resulting in an amputation to the finger.

An employee was checking the belt tension by pulling the belt toward the pulley on the motor. As he pulled on it, the belt started to turn faster than the employee expected and his left little finger was caught between the high-tension belt and the pulley on the motor. He pulled his hand out of the pinch point, which caused his finger to scrape against the edge of the pulley. The employee sustained a severe laceration and amputation to his left little finger.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was unloading frameless mirrors from a trailer. When unstrapped, the mirrors fell over onto the employee, who suffered a skull fracture, a brain bleed and blood clot, a broken left ankle, and lacerations.

The employee was checking an airlock to determine whether there was suction when their left index and middle fingers were pinched between the rotating rotor and the housing of the airlock. The employee suffered amputations of both fingertips without bone loss.
