On June 20, 2025, an employee was grabbing boxes from a conveyor and staging them on a pallet. While they were picking up a box, the employee's right hand was caught between the roller and the belt of the conveyor line, resulting in a right ring fingertip amputation and a laceration to their right middle finger.

On November 6, 2017, an employee slipped and fell while walking. The employee dislocated the left wrist while trying to brace for the fall, requiring hospitalization and surgery.

An employee working at a conveyor line was trying to remove a can from underneath a pallet. A forklift driver did not see her and delivered the next pallet, which pushed the pallet that was staged on the conveyor. The employee's ankle was pinned between the pallets and a support beam on the conveyor, and she suffered compound fractures to her tibia and fibula.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

A production maintenance technician's right index fingertip was severed after contacting the valve of a filler machine on a production line. His right index finger was partially amputated below the nail.

On August 14, 2025, an employee was priming a pump that was failing to pump mustard. The employee took a water hose and started water into the pump. The employee's finger slipped off the nozzle of the hose and came into contact with the pump's impellers, causing the employee to suffer an amputation to the right index finger at the first knuckle.

An employee was cleaning packets from a box shaker when the shaker crushed his left middle finger. The employee sustained partial amputation of the finger through the phalanx.

An employee was cleaning inside a machine using a rag. The rag was caught in a sprocket and their hand was pulled into the machine, resulting in the amputation of two fingers.

An employee was shrink wrapping product on the hand shrink wrapper. As the employee went to tear the shrink wrap, his finger struck a sharp edge on the conveyor stand. The employee's fingertip was amputated.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
