An employee was performing maintenance on a spiral conveyor. As the employee cleaned excess lubricant away with a rag, the chain on the conveyor caught the rag and pulled in the employee's right hand. The employee suffered an amputation to the middle finger.

As he backed out of the truck onto a dock, with the papers on a pallet carried by an electric pallet jack, the truck rolled away from the dock and he fell to the ground. The pallet jack and the pallet with papers fell onto him. He suffered a broken left tibia and a left ankle injury.

An employee was backing into an industrial lift on the main floor with a cart of books. The lift was not on the main floor and he fell 12 to 13 feet down the shaft, with the cart of books landing on top of him. He suffered bleeding on the brain and a broken shoulder, and he required stitches to the back of the head and hospitalization.
