On January 28, 2022, an employee was removing an extra bar of steel stock that was fed into a bar stock feeder. The bar of steel stock fell approximately 1 inch onto the employee's left ring finger, resulting in a partial amputation.

Mercury Marine

An employee was assembling the gearbox of a lower unit (of a boat motor). They used a handheld electric drill to power the lower unit's gears and inspect the assembly. A gear was out of tolerance, so the employee placed sandpaper in the gearbox to remove any burrs or other material. The employee activated the drill and their left middle finger contacted a nip point between the gears in the unit, resulting in a fingertip amputation.

An employee was walking in the racking area to check parts when they were struck by an oncoming forklift. The front wheel ran over the employee's right leg, resulting in fractures to the leg and ankle.

An employee was pushing an electrical pallet jack loaded with parts baskets. While going through a doorway, the parts baskets caught the door frame and fell back onto the employee, who suffered fractures to the skull, cervical vertebrae, and ribs, as well as an ankle injury.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

KP Building Products, Inc.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

BJ'S WHOLESALE CLUB, INC.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

International Steel and Counterweights LLC

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

Seneca Mechanical, LLC

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

Seoyon E-HWA Manufacturing Auburn, Inc.

An employee was making repairs to a milling machine located in the mold shop. While using a screwdriver to make adjustments, the machine energized and struck the employee. The employee's right arm was fractured.

TAIHO CORPORATION OF AMERICA, INC.

An employee was breaking down dunnage to prepare totes and trays to go through a tote washer. A tow motor driver asked the employee to move a cart that was in the way so the tow motor could remove a gaylord. The employee moved the cart and then stepped away from the tow motor. As the employee moved to an area between carts, his right foot got caught by a cart, causing him to twist and fall on top of a cart, landing on the left flank of his torso. The employee suffered a hemorrhage on his left flank.

An employee tripped over a computer cord and fell fracturing their femur.

Three employees were installing a transfer rail in a press. There were two employees holding the rail and the injured employee was at the end of the rail guiding the rail into the bracket. The task was done manually with no mechanical equipment. The employee had their finger at the end of the rail and went to make sure the rail was lined up while the other two employees pushed it into the bracket, causing their finger to become pinched between the rail and bracket. The employee's middle fingertip was amputated. Protective gloves were worn at the time of the incident.

An employee was walking across the dock, interacting with forklift drivers on what was being loaded. A forklift drove forward with a tall load on the forks. The employee was struck by the load (running board/rocker panel rack) on the forklift. The employee sustained several contusions and an injury to their right Achilles tendon that required surgery.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

Asher Enterprises, Inc.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
