An employee was in a cage hoisted by a forklift, taking juice samples on top of a tanker truck. When she got off the tanker, the cage got caught on the metal on top of the tanker. The cage loosened and the employee fell to the ground with the cage. The employee sustained a broken left jaw, broken collarbone, broken left arm, broken wrists, and a right eye injury.
