An employee was starting equipment with rollers that normally rotated in a outward motion to move stock away from the rollers, but had become a pinch point after maintenance. When the employee reached for stock, their left middle, ring, and little fingers got caught in the pinch point, resulting in amputations to the first knuckle.

As he was about to tighten a metal screw with a metal screwdriver, he came into contact with the metal frame of the disconnect. The resulting arc flash caused second-degree burns to his right hand and face, as well as eye irritation.

At the feed mill, an employee was wrapping red duct tape around a metal trough. His left index fingertip went inside the trough housing through an unseen hole in the metal. The conveyor screw inside the trough caused a partial amputation of the left index fingertip.
