An employee was starting equipment with rollers that normally rotated in a outward motion to move stock away from the rollers, but had become a pinch point after maintenance. When the employee reached for stock, their left middle, ring, and little fingers got caught in the pinch point, resulting in amputations to the first knuckle.

An employee was doing electrical preventative maintenance on a motor control center. As he was about to tighten a metal screw with a metal screwdriver, he came into contact with the metal frame of the disconnect.
