Final narrative
An employee was monitoring the air-lock device that is used to move flour product to a different area of the facility. The air-lock machine became choked with product. The employee unscrewed a section of the housing and proceeded to unclog the machine. The machine activated while his right hand was in the point of operation resulting in amputation of the index finger to the first knuckle and lacerations to the middle and ring fingers.

An employee was doing electrical preventative maintenance on a motor control center. As he was about to tighten a metal screw with a metal screwdriver, he came into contact with the metal frame of the disconnect. The resulting arc flash caused second-degree burns to his right hand and face, as well as eye irritation.
