An employee jumped up on the concrete floor to retrieve an empty 5-gallon bucket from on top of a storage cabinet. When he landed, his right femur fractured.

On January 6, 2025, at approximately 11:56 PM, an employee was clearing products from the packing table of a trim press when their left middle finger came in contact with the die. The employee sustained amputation of their left middle finger pad.

An employee was removing plastic shards from a machine using a compressed air hose. She stepped on the guardrail to gain better access to the accumulated plastic and her hair was caught in the rotating canopy drive shaft. The employee suffered hair loss and scalp damage. The machine was not guarded at the time.

An employee's face struck the handle on a machine resulting in chipped teeth and a laceration inside the mouth.

A temporary employee was stepping on a scrap plastic sheet/formed cup to flatten it out. The employee slipped on the sheet and fell to the concrete floor, suffering a broken left wrist.

On October 25, 2023, an employee was in the warehouse driving a forklift toward the dock door. The forklift struck a conveyor belt and the employee sustained broken ribs.

An employee was operating an electric pallet jack when it struck a stanchion. The employee sustained fractures to the right chest and right ankle.

An employee was using a walkie-rider to load a trailer. As she turned a corner, the walkie-rider struck the trailer and the employee was thrown to the ground. She was hospitalized with a fractured left leg.

An employee was driving a forklift and struck a pole, resulting in a broken left ankle.

An employee was removing a bolt from a caliber on a machine with one foot on the platform and the other foot on the piece of equipment. As he went to loosen the bolt, he lost his balance and fell four feet to the ground.

An employee was jogging a loose belt on a machine. The machine's sprocket and belt caught the employee's right hand and lacerated the top of it. The employee was hospitalized.

A pallet had been loaded onto a trolley to be picked up for wrapping, but a broken bottom board on the pallet was blocking an entry roller sensor, tripping it and preventing the trolley from moving. An employee removed the board and the trolley began to travel. Her left knee and thigh were lodged between the framing of the trolley and a stationary pallet insert line. She suffered a laceration to the back of the thigh.

An employee put a roll of lamination film on a lamination machine to laminate (glue) two pieces of film together. When he pushed the button to disengage a roll from the machine, the empty roll moved over and his right middle finger was caught between the metal rod and the core of the laminate roll. The fingertip was amputated.

An employee was adjusting a piece of cardboard behind the draw rolls on a bag machine. The rolls caught and crushed the employee's right index finger. He was hospitalized and the finger was medically amputated.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.
