An employee was operating an electric pallet jack, delivering pallets of groceries to a store. The employee was walking backward when her foot was pinned between the pallet jack and a video pole, resulting in lacerations and fractures to her ankle and heel. She was hospitalized.

An employee was unloading pallets of product from a trailer when a plastic-wrapped pallet became stuck against the bulkhead of the trailer. The employee was dislodging the pallet when he lost his grip and fell backward, striking his head on the side of the trailer. He then tripped over the forks of a pallet jack and fell to the floor of the trailer. The employee was hospitalized with a brain bleed and pain in his lower back and ribs.

An employee was cutting boxes with a safety knife. The safety guard on the knife broke off and the knife lacerated his left forearm. He was hospitalized.

An employee was on a catwalk five feet above the ground to see into a refrigerator trailer engine. As the employee was adjusting the belt on the engine, they fell off the catwalk to the concrete below, resulting in a left hip fracture. The employee was hospitalized.

An employee was operating a stand-up forklift in the warehouse when it struck another forklift. His left foot was caught between the two forklifts. The employee was hospitalized with a foot fracture and lacerations that required surgery.

An employee was walking to a customer's door to deliver a package when they tripped on uneven pavement and fell to the ground. The employee sustained a hip fracture.

An employee was leaving work for the day when he tripped over mulch and fell to the ground. He was hospitalized with a fractured hip.

An employee was walking on the sidewalk toward the front entrance of the building to start her work shift. She tripped over uneven pavement and fell onto the concrete sidewalk. The employee was hospitalized with a fractured left hip and a sprained left knee.

An employee was walking into work through a doorway when she tripped on the door sill and fell to the floor, fracturing her humerus.

An employee was preparing to remove leaf spring bushings on the front drive axle of a tandem axle tractor. After raising the axle with a hydraulic jack, he placed two jack stands under the rear frame rails of the tractor and placed additional jack stands under the front drive axle. He then repositioned himself at the rear of the tractor where he reached for the release valve of the jack to begin lowering the axle (and frame) onto the jack stands. He was in a kneeling position while using his right hand to operate the release valve. His left hand was caught between the lowering frame rail and a jack stand. His left ring and middle fingertips were crushed, resulting in amputations.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
