On March 24, 2022, an employee was load-testing an air dryer when the muffler failed and flew apart. It struck the employee in the head, causing three facial fractures. The employee was hospitalized.

An employee became dizzy while on an elevated surface, 6-8 feet high. He began to climb down a ladder. The ladder's feet slipped out and the employee fell to the ground, suffering a broken right fibula and tibia.
An employee was working on a 60-kilowatt diesel generator in an outside workshop. He noticed some squeaking while checking the belts on the engine. As he tried to spray lubricant on the belt, the fan caught his right index finger, causing a large laceration. The finger was medically amputated above the second knuckle. The fan was not locked out/tagged out at the time of the incident.
An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.
