An employee was standing on top of a plastic pipe rack, helping another employee put pipes into the rack. The pipes were being lowered by a forklift, and as the two employees were trying to get them unstrapped the forklift boom struck the first employee's toe. The toe was broken and the bottom of it was medically amputated.
