Final narrative
A contractor was delivering an oversized item (AV equipment) weighing approximately 350 lbs. As the two delivery people were moving the item through the entrance of the building, they lost control of it and an employee moved forward to assist. The item fell on the employee's legs and knocked him to the ground. The employee sustained torn tendons (ACL) in both knees.

On April 29, 2021, an employee was in the gym, trying to keep a child from running out of the area. She fell to the floor and broke a bone in her lower left leg. She was hospitalized, requiring surgery.
An employee was attempting to change a broken fan belt in an HVAC system when his finger was caught and lacerated between the belt and a motor puller, requiring surgery.

An employee was climbing a fixed ladder to perform maintenance in an attic. While unlocking the attic entry, the employee fell from the ladder more than 10 feet to the concrete below. The employee suffered a fractured hip, pelvis, tail bone, arm, and elbow, resulting in hospitalization and surgery.
An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

As she turned to set the container down, it slipped out of her hand and landed on the grill. Butter splashed up and hit her in the face.
